First things first, obtain an application form from your local WV Lions Club. The clubs will decide if the applicant falls under the guidelines for assistance. The WVLSCF follows the US Poverty guidelines. Click here to find out what they are.
After filling out the application, the club determines if the individual qualifies for assistance. The application is then sent to the institution where care will be provided. Currently the West Virginia Lions Sight Conservation Foundation is in partnership with the West Virginia Academy of Eye Physicians & Surgeons, The WVU Eye Institute, and doctors and hospitals throughout the state that perform surgeries.
The doctor and hospital will set up appointments with the individual.
